Please read and sign this policy form prior to beginning Pilates and/or Hoop Dance classes. You only need to fill this out once to establish a client relationship with Fusion Movement.

Pre-registration is required to reserve private lessons and also to reserve a space in a class. Pre-registration involves payment in advance as well as filling out paperwork. Paperwork is only required for new clients and will only need to be filled out once. Please update your paperwork if there have been any significant changes to your health and please inform Susan personally of any health changes.

Fusion Movement gives refunds in the form of credit for canceled private lessons when 24 hours notice has been given. Your credit will be valid for three months from the time of the canceled lesson. Fusion Movement DOES NOT give monetary refunds for canceled private lessons.
Refunds and/or credit ARE NOT given for missed classes. If space allows you have the option to attend a different class to make up for a missed class during the time frame of the 6-8 week session that you are currently registered for.
If you are registered for a class series that is full and you decide to drop out of it prior to the start date you WILL NOT receive a monetary refund UNLESS your spot can be filled.
If you are registered for a class that is not full and you decide to drop out of it you must give 72 hours notice prior to the start date of the series in order to receive a refund. If you do not give 72 hours notice you WILL NOT be refunded any money.
If you drop out of a series once it has started you WILL NOT be refunded monetarily and you WILL NOT receive credit.
Refunds for hoops will be given in the form of credit with Fusion Movement IF the hoop is returned in the exact condition that it was purchased in within 30 days. You must have a valid reason for returning the hoop. Refunds will not be given for any damage accrued due to use once the hoop has been purchased.
